LIABILITY RELEASE.
LIABILITY RELEASE: DEUX Pilates LLC
Please read the following terms carefully. By participating in any in-person or virtual session provided by Deux Pilates LLC, you consent to, and agree to release Deux Pilates LLC from liability according to, these terms. Unless Deux Pilates LLC notifies you otherwise, this waiver and release will apply to all sessions undertaken by you with Deux Pilates LLC.
You acknowledge that:
- you are over 18 years of age;
- you understand that the sessions are a form of physical activity and that there are inherent risks in undertaking any form of physical exercise;
- you have disclosed, or will disclose to Deux Pilates LLC prior to the session commencing, any pre-existing conditions that may place you at a higher risk of injury or inhibit your ability to participate in a class, this includes, but is not limited to:
- injuries, pain or recent surgery;
- pregnancy or if you have recently given birth; or
- respiratory or heart conditions or high blood pressure.
- you will notify Deux Pilates LLC if you experience any pain throughout the session and will immediately stop participating in the session if requested by the instructor to do so;
- based on information you provide, Deux Pilates LLC may decide not to allow you to participate in a session, or may require you to provide a medical certificate affirming that you are able to participate in a session, in the instructor’s sole discretion;
- you are solely responsible for ensuring that your physical environment is suitable for your participation in any session
To the extent permitted at law, you release and hold Deux Pilates LLC (and where applicable its, directors, employees and representatives) harmless from any liability, cost, expense, damages or claims (including claims of negligence) arising from or which may be suffered or incurred in connection with your participation in any sessions provided by Deux Pilates LLC.
I have read and understood Deux Pilates LLC’s policy:
- All classes must be prepaid
- There is a strict cancellation policy: All lessons require cancellation 24 hours before the scheduled lesson. Cancellations can be made by email or cancelled directly on the scheduling site. Canceling less than 24 hours prior to the scheduled session is considered a Late Cancel and your session will be forfeited.
- If you are 10+ minutes late to a private lesson without notification, it is the instructor’s discretion to cancel the session as a No Show
- All sales are final. No refunds, exchanges, or transfers.
- Liability waiver must be completed online or in person prior to first lesson.
Consent: I acknowledge to the best of my ability, that I am in good health and have no known medical problems that would restrict my ability to participate in this exercise program.
